To Whom It May Concern:

On behalf of "THE CARROLL COX SHOW" I am requesting your assistance in obtaining information relating to the recent quarantining of a female in her early twenties who tested positive  for the H1N1 flu virus.  The affected  person is a teacher at the United States Job Corps, located at 41-467  Hehemanu St. Waimanalo Hawaii.    For privacy reasons we are withholding the name of the affected party.   I would appreciate your assistance in obtaining answers to the following questions:

When was the affected person diagnosed for the H1N1 virus?

When was the affected party quarantined by the State Health Department (DoH)?

How long will the affected party be quarantined?

Has or will the State Department of Health notify the public of this case?

Has the DoH confirmed other cases of H1N1 at the Job Corps site this year?

Is the DoH monitoring the Job Corps staff and students?

Is the DoH requiring immunization of the Job Corps staff and students?  If yes, when did that begin?

Again thank you for your assistance in this matter.   I would appreciate a response by the end of business, as I want to post information on our site CARROLLCOX.COM.

Thank you,

Carroll Cox
